Information Concerning Your Outpatient Procedure
This letter is to inform you of all possible fees that are could occur because of your planned procedure. These are in addition to the charges submitted to your insurance by your surgeon, Dr. Graham Sellers, Dr. Craig Brown, Dr. Warren Strutt or Dr. Noelle Bertelson.
- Medical facility – This is the hospital or surgery center where your procedure will take place. You will see separate charges from the facility submitted to your insurance. Co-pays and deductibles, as outlined by your plan, will apply.
- Anesthesiologist – If anesthesia is necessary or for your planned procedure, you will see separate charges from an Anesthesiologist submitted to your insurance. Co-pays and deductibles, as outlined by your plan, will apply.
- Pathologist – If your planned procedure results in tissue removal or biopsy, the sample will be submitted to a pathologist. You will see separate charges from the Pathologist submitted to your insurance. Co-pays and deductibles, as outlined by your plan, will apply.
- Special Information regarding Colonoscopy – If your colonoscopy is scheduled as a screening and the doctor discovers a polyp, it will be removed. Your procedure, by many insurance companies, will now be classified as a “diagnostic exam”, not a “screening exam”. Please be aware that with some insurance companies this change may affect your out-of-pocket responsibility for the procedure.
- Screening exams are classified as colonoscopy exams for persons age 50 and older who do not have a personal history of prior polyps or cancer, and do not have active signs or symptoms of potential disease. If you have had a prior colonoscopy with polyps or cancer found you may NOT qualify as a “screening exam” for future examinations. Check with your insurance company to see what their guidelines are.
Our office will pre-authorize your planned procedure, if required, with your insurance company. However, the portion, you, the patient, are responsible for is determined by your insurance plan.
